Release of Liability Form for In-Person Customers:

All in-person lesson customers  (Individual/Couples/Groups and On-Course lessons) are required to agree to & sign (in the form of booking an appointment with us) our Release of Liability form as part of our scheduling & payment check-out process. **By scheduling an appointment with us, you are voluntarily signing and agreeing to (scheduling an appointment counts as signing) the terms and conditions of our Release of Liability Form and agree not to hold Premier Golf Lessons liable for any illnesses, injuries, slip & fall accidents, or any other involuntary injury or accident that occurs during your lesson with us.
